(NOTE:  For PTAC Program this information must be submitted by the Program Manager on record with DLA.  Employees submitted by Program Managers must, as a general guideline, be on your DLA SCAA Roster.  This information may be verified by APTAC headquarters or the membership committee for accuracy and is subject to final approval for membership by the APTAC Board of Directors.  Also, please remember to update your roster when employee changes take place during the year.  These must be submitted by the Program Manager also.)
